    Dr Mariano, numerous times you have said, “Once Testosterone is in a good range, then any problems that are left have other causes. “

    But my question is, how many weeks, months, years do you give the individual who was previously hypogonadal and brought back to 650+ Total Testosterone before making the judgment that problems are not due to testosterone?

    The effects of testosterone are fairly rapid. Generally, if low testosterone was the only problem, then a patient should show improvement within a week of restoring testosterone level.

    Generally, if there are still problems after three months of treatment (to allow for cascade effects of testosterone on other signals and metabolism) then one should start look for other causes.

    Generally, testosterone deficiency is never an isolated problem for the patients I usually see. So I automatically try to identify as many causes of the patient’s problems as I can to help determine the entry points for treatment.

    Other issues should be evaluated at the same time. As the person is on therapy, the health professional should be correcting the other imbalances. With in 4 weeks with proper testing you should have an idea of what is going on then how to address things appropriately. This is why with with Drs I use a layered approach. Cases I handle are not your typical run of the mill TRT cases, but rather referred by drs with patients who have been to 50 plus specialists, $100,000+, 5-10 year with no resolutions.
