    I’take HRT with testosterone gel.

    My total and free testosterone is optimum level but my DHT is still very high. I don’t loss my hair, no skin oil… (perhaps because I have partial DHT insensitivity), my libido is fine.

    What the risk to have high DHT ?

    As far as I know, Dihydrotestosterone (DHT) when too high poses some problems:

    male pattern hair loss in men with high scalp 5-alpha reductase levels
    excessive body hair
    acne

    Isn’t there a relationship between high DHT retroperitoneal fat and a relationship to insulin (perhaps increased IR risk ?) ? Off the top of my head, I seem to remember reading about this. I also read an article by Patrick Arnold last year where he wrote that in muscle tissue, DHT isn’t involved or is marginelly active, whereas as an androgen DHT has much to do in CNS signaling. Furthermore, I seem to remember that testosterone isn’t very much active in CNS signaling but is very much in muscle tissue.

    My DHT has been consistently elevated, and I have vertex MPB to show for it. Very little acne in my back, none on my chest, and zero on facial skin. As a teen however, I had a severe case of cystic acne (back, face, chest) from age 14 up until 24 years old. It was tough seeing my peers hanging out with girls, while I had to keep to myself.

    Systemic dht doenst have any value and it does certainly not mean that you will find the same amounts regionally, for example in your hypothalamus, your prostate, your skin or wherever you are referring to. Measuring DhT in blood is by far useless, unless we are talking about extremely low levels (like in androgen insensitivity after GnRH stimulation) or extremely high – dont know of any such syndrome !! In functional situations, it doesnt have any use though. If you could cut out your prostate or pituitary and measure the dht levels there then that would really be usefull. Very complicated hormone metabolite to discuss about.

    Why do you mention you have some partial insensitivity though, i don understand that?? How did you find out about this???

    Partial insensitivity ?

    In my case, my serum DHT is consistently elevated to downright off the chart. I have vertex MPB, denoting I have elevated SRD5A1 activity in my scalp.

    What I made reference to is that I seem to recall having seen a relationship between high DHT and insulin resistance and increased retroperitoneal fat deposition.

    You probably do not have the genetic makeup for MPB, as it seems to have much to do with genetics, even though DHT has been pointed out as one of the major culprit. Scalp 5ar type 1 is most likely determined by the MPB gene.

    DHT isn’t the devil, as it acts to keep E2 in check.

    From the more recent studies I’ve seen, DHT helps reduce visceral fat (belly fat) and reduces insulin resistance.

    Testosterone can be considered a precursor for DHT in the brain. But both testosterone and DHT can act on testosterone receptors.

    In muscles, DHT does not signal for muscle hypertrophy. Testosterone signals for muscle hypertrophy.

    DHT in the body helps determine secondary male characteristics including body hair.

    What if one has MPB but also has diffuse thinning all over the rest of the head, I guess described as low quality hair that grows in different directions and is diffuse and thin and straw like.I have this, I have MPB in my family but also have diffuse low quality hair on the womens side also. I even have light shaded areas that are very diffuse so not patchy but missing hair and straw like hair that sticks straight out even when short enough to be stubble. I was wondering if this could be caused by any other hormone issues? I have used steroids in the past as a crutch for my unknown hormonal issues as a both mental and physical crutch, and was wondering besides speeding up my possible propensity for MPB if the stress of my extremely grueling work out regimen over the past years has exacerbated some non MPB issue enough to affect hair quality.
