    33 yr old male 250 lbs 6′ , on trt for 9 yrs now on and off , been on this run for 8 months , 100 mg test depo per week .

    main concerns are very itchy and puffy left nipple . hot flashes , flight fight feeling all the time , high anxiety , just over stimulated !!!

    Low Vitamin D and b12 (need to supplement the first for 10,000iu and the second with a lozenges 1000mcg a day)
    Low progesterone which may indicate a low pregnelone
    Signs of insulin resistance with a 101 glucose

    as for the rest its clear that your medication in pumping your testosterone isn’t effective

    I prefer fasting glucose to be between 93-100 so long as there is intact gluconeogenesis and minimal insulin resistance.

    Testosterone cypionate at 100 mg a week should result in good trough levels of testosterone – around 650 ng/dL. If it is less, then perhaps the half-life of testosterone cypionate is short and a dose adjustment needs to be done.

    Ideally, from my point of view, TSH should be below 1.0, preferrably between 0.03 to 0.5. Not everyone can tolerate this however because of excessive pro-inflammatory signaling interacting with thyroid hormone.

    Anxiety, irritability, insomnia, hot flashes indicate an excess in sympathetic nervous system activity. Outside of external stresses, this may indicate a compensatory response to metabolic problems or impaired ability to produce sufficient control signals for norepinephrine. These include serotonin, dopamine, GABA, testosterone, progesterone, estradiol, etc.

    Generally, growth hormone production and signaling may be impaired by multiple other signaling problems including lack of adequate thyroid hormone, testosterone, DHEA, etc. Substances that increase liver inflammation – such as a single drink of alcohol – knock out growth hormones major effects.

    Nutritional optimization is important to improve metabolism and signaling. Low vitamin D and B12 occur often when people avoid animal fat and internal organs like liver in the diet. These ingredients are part of traditional diets which are highly nutritious compared to diets in the modern era.

    I prefer doing a plasma fractionated catecholamine test to the urine test since it is much more easily correlated to nervous system stress levels.
