Hi Dr. M,
I’m wondering what you think about the dangers of Accutane and whether any specific treatment options are available for those who have used it in the past.
I used it over 15 years ago and it was at that point that I started to have hormonal issues.
As you may know, Accutane directly affects cholesterol metabolism (patients are required to monitor their lipids while using it) and it has been linked to suicides.
For those people who have used this drug in the past is there anything you recommend they discuss with their doctors?

Dr M is the expert, and I am in no way trying to intercept your question nor am I any authority on the subject, I am just a regular “member” on this site who wants to better my own health and learn from Dr Mariano and others on this site. So here’s my two cents. You don’t mention why you want or need Accutane. If it is for Acne, then I can only say that from my own experience, when my Dermatologist discussed the idea of rx’ing me it, it seemed to be “More of a hassle” to obtain it and he wanted me to get written permission from a Psychiatrist because of the concerns about the possible side effects it may have on people taking anti depressant meds and the SUICIDE issue that you mentioned in your question to Dr M. I personally did not want to go thru all the hassle that it seemed to have to go thru to get it, and after I had researched Accutane more in detail, to be honest with you, It kind of scared me a little bit after reading about it’s chemical make up and all of the other things it can possibly do to a person. I chose to go with an anti-biotic med and a topical gel both , rx’d by my dermatologist and it worked out fine for me. But as you know already I’m sure, everyone is different in so many ways !
Just a little of my own personal views on Accutane. Just trying to help out with a quick reply as I just so happened to have had a few minutes to add this quick reply. Hope it helped you somehow ? Geno
February 26, 2010 at 6:27 am #4174bridgerMemberI used it last year for acne, which i’ve only had since starting TRT 3 years ago. I tried a few different oral antibiotics and a few topicals as well with little benefit. Accutane was the last resort. It was a pain, had to follow strict guidelines to get it filled. Monthly bloodwork, monthly check up with derm, had to get script filled within a certain amount of days. However, the guidelines are much more strict for females. Anyway, it worked better than the other things i tried for acne, but it has returned somewhat. My joints and body ached constantly and just felt run down for most of the treatment. However, I am bipolar and did not have problems with depression. I tried to be very aware of how i felt and saw my therapist regularly. I also tried to exercise and eat healthy which probably helped my mood. Physically i felt like shit and couldn’t wait for it to end, but as my face and shoulders became acne free it gave me a psychological boost.
One thing I asked my dr about was back pain. I never really had lower back pain until i used this drug. He said the side effects go way once the drug is discontinued. It didn’t for me. Its not severe, but not something I had before. He was an older doctor and didn’t seem up to date on the most recent studies, said there weren’t any studies using a low dose accutane showing it to be effective. There actually was at least one at the time I asked him. So, I would bring in that study and suggest it your doc. It is supposed to be as effective as higher doses with less sides.

Accutane is an effective treatment for severe, treatment-resistant acne. In my experience, the patients in my care haven’t had serious problems when they were simultaneously treated for severe acne by their dermatologist with Accutane and myself for their mental health problems.
Severe acne is disfiguring. It may impair a person’s ability to socialize and form relationships. It may cause the affected person to develop poor self-esteem and depression or other mental illness. This, itself, increases the risk for suicide.
Accutane has significant risks. These include adverse effects resembling Vitamin A toxicity. This includes birth defects. Accutane also, infamously, has a significant risk for contributing to depression and suicide.
Thus, it is important for patients to discuss all relevant issues with their physician – including any prior history of mental illness, to discuss how adverse effects will be handled (including the risk for depression and suicide, and how treatment is going to be monitored.
Already, there is a strict program physicians and their patients have to use in order for the patient to receive Accutane.
Accutane may be medically necessary as a treatment in some patients. Given the risks, safety considerations and risk management are very very important.
September 7, 2010 at 7:04 pm #4175accutanedMemberAccutane is a nasty drug that has been taken off the market due to all the personal injury lawsuits associated with it. The generics are still available however. Accutane is also a 5-AR inhibitor just like Finasteride and can cause horrendous permanent physical, mental, and sexual side effects. Accutane and Finasteride are different substances but they do share similar mechanisms and both work as permanent anti-androgens. Accutane can also cause immune system related disorders and neurological related side effects. I do realize that my extreme suffering caused by Accutane is in the minority so I do feel that patients with extremely severe acne should have the choice to take the drug, but I do not see how these risks could be properly conveyed to the public. Accutane has also been associated with severe gastrointestinal disorders resulting in the removal of one’s colon/large intestine. I do want to see Dr.Mariano in the near future but would like him to see the possible outcomes of any 5-AR inhibitor usage, however ‘rare’ they may be, Accutane included. I am very pleased with this website and want to thank Dr.Mariano for his very helpful information that could possibly alleviate my long term Accutane side effects.
